Welcome to DHU Health Care CIC self-referral platform for the Covid Medicines service that triages and treats patients in the community who have tested positive for Covid.
If you feel you are experiencing a life-threatening emergency please do not proceed and call 999 immediately for further assistance.
If you can answer yes to all the following questions then please proceed to complete the form:
- Have you tested positive for Covid in the last 5 days?
- Have you had symptoms that have started in the last 5 days?
- Are you unwell with your symptoms of Covid?

If you do not answer yes to all the above questions but are unwell and need further assistance please contact your GP surgery in hours and the 111 service when your GP surgery is closed.
Upon completing and submitting the form you should be called back within 24 hours by one of our Covid Medicines Delivery Unit (CMDU) clinicians. They will carry out and assessment and determine if treatment is required. Antiviral medicines may not always be needed in cases where the patients’ symptoms are mild.
If a call back has not been received within 24 hours of submitting this form please call the NHS 111 service by dialling 111 from your phone.
